First Visit
Your first appointment with me may take 15 to 20 minutes this will allow me to obtain a comprehensive history of your injury and condition, as well as your medical history. I always do my best to run to time and appreciate your time is as valuable as mine, however, there may be unexpected circumstances arise that cause me to run late.
I would like to give you some important details on my practice. If you have any questions, queries or concerns throughout your treatment with me, please do not hesitate to contact Hayley, my Personal Assistant on 9230 6333 or hayley@coastalorthopaedics.com.au and she will be more than happy to assist you or point you in the right direction to get the right answers for you. WHAT TO BRING TO YOUR APPOINTMENT?
Referral: Please email or fax a copy of your referral to our office prior to your appointment. If this is not feasible for you, simply bring it along on the day of your appointment.
Cards: Medicare, Pensioner, DVA & Private Health fund card, workers compensation details including Insurer and claim number.
Radiology: Please provide Hayley with details of any recent scans you have had including MRI, x-ray, ultrasound or CT. This will allow me to ensure I have access to these electronically prior to your appointment.
Clothing: If your problem is regarding your knee, please bring shorts along with you. If your problem is regarding your upper limb (shoulder, elbow or wrist) a sleeveless top/singlet would be Ideal.
PATIENT REGISTRATION SHEET
To ensure we can provide you with a pleasant, streamlined experience when attending our clinic if you can please complete our patient information sheet, privacy consent and return this, along with your referral, to our clinic prior to your appointment, that would be greatly appreciated. If this isn't possible, simply arrive 10 minutes earlier than your appointment to fill in the required Information.
